Integrating Google Forms and Slack
Send message to Slack channel on Google Form Submission
You can integrate Google Forms with Slack using Form Director. Form Director enables you to connect Google Forms with Slack, allowing you to send messages based on form responses.
When a user submits a Google Form, the responses are automatically sent to Slack accordingly.
Form Director is a Google Workspace add-on that directs data submitted from Google Forms to applications like Google Docs, Forms, Sheets, Contacts, Calendar, Tasks, Email, Trello, Slack, QuickBooks etc.
Installation:
It is available in the Google workspace Marketplace.
Please check installation instructions to install the add-on.

Choose Slack from the list of apps
Name your service
Choose Send Message under services
Click on "Add to Slack"
Click on Allow to allow Form Director to access your Slack account for reading your Slack Channels and send message as Form Director
Choose the channel from list or provide the channel id to send the message.
Select the required Message Suffix.
Note: When it takes a long time to load or retrieve channels in Slack, utilizing channel IDs in the integration can help. Instead of relying on the standard channel list, you can directly c specific channels by providing their unique IDs.
Open any web browser and log in to your Slack account.
Now, go to your workspace main page and view the URL in the search bar at the top.
The URL looks ends with a C and letters. This part of the path represents your Slack Channel ID.
To use private channels with the Form Director Integration for Slack, you must go into each private channel in Slack and add the Form Director app.
Follow the below instructions to Form Director to add the app to a private channel:
Go to your slack account. Select the private channel and right click on it and then select View channel details option.
In the channel details, select Integrations. There you will find an option "Add and App" in Apps section. Select it.
Search for "Form Director" to locate the Form Director app. Click "Add" to add the app to your private channel
The Form Director App will be added in your Private Channel.
Note: You must repeat this process for each private channel that you would like to use with the Form Director integration of Google Forms and Slack.
Now your private channel will get listed on Form Director Add-on.
Click Setup Mapping and setup the mapping.
Mapping of form fields to that of app fields is to be done in order to send messages on form submission.
Click on the add icon to add new mapping
Once you have done mapping you can save the service
Note : Mapping should be done between app field and form field of same type.
Notify your audience with success or error notifications.
Switch on Send Email to send email during execution.
Send your own customized emails on form submission to your audience.
Select the attachment to email from file upload in Google Form.
Click Pick Drive Files to attach files to Email.

You are all set to send message to Slack channels on your Google Form submission
Submit the Google Form
You can see the message appearing in the selected Slack channel
